You may register here on the website or by printing out and mailing the PDF registration form below.  If you use the PDF form, you can simply mail your payment with your form.  If you use the online registration, you should select a payment method from the Payment Options section below.

can return to us via mail if you register online. 

To register by mail, please click the button below.  You will be directed to a  PDF form you can print, fill out and mail along with your payment (Note: This document includes a registration form and three installment payemtn forms.  Print only the first page if you will be sending your full payment with your registration.
